Follow the instructions via the link on your decision letter. You will not become an official UCLA graduate student until this form has been completed and processed.

- Submit Waiver of Liability, Assumption of Risk and Indemnity Agreement
- Sign University of California Consent for Photo/Video/Recording Release

Center for Accessible Education
The Center for Accessible Education serves thousands of our undergraduate, graduate, and professional students enrolled at UCLA. Students seeking reasonable accommodations or support services from The Center for Accessible Education should contact their designated program manager to help register with the office as early as possible so that you can work closely with your Disability Specialist and tap into the knowledge and support available to navigate your UCLA experience.
Health Insurance & Immunizations
EMBA students are automatically opted out of the UCLA Health Insurance for students. Therefore, providing immunizations and health records are not required as part of the admissions process for Executive MBA students. You may ignore the immunization requirement submission listed on myUCLA.
While immunizations are not required for EMBA program enrollment, the UCLA Health Arthur Ashe Student Health Center highly recommends that each student have the immunizations listed on the website here. Click to view the immunization exemption to see the confirmation that our EMBA program is exempt from submitting the immunization records.
